Schistosomiasis disease, also known as
bilharzia, snail fever, and Katayama fever, is a disease caused by parasitic
flatworms of the Schistosoma type. It is transmitted by contact with
contaminated fresh water (lakes and ponds, rivers, dams) inhabited by snails
carrying the parasite. Swimming, bathing, fishing and even domestic chores such
as laundry and herding livestock can put people at risk of contracting the
disease.</div>

The disease is especially common
among children in developing countries as they are more likely to play in
contaminated water. Other high risk groups include farmers, fishermen, and
people using unclean water during daily living.</div>

Larvae emerge from the snails and
swim in the water until they come into contact with an individual and penetrate
the skin. Once inside the body, the larvae develop into male and female worms
which pair up and live together in the blood vessels for years. Female worms
release thousands of eggs which are passed out of the body in the urine and faeces.
If people urinate or defecate in bodies of freshwater, the eggs migrate to
snails where they eventually hatch and begin the cycle again.</div>

Some Schistosoma eggs, however,
remain trapped in the body and migrate to specific organs (depending on the
type of parasite) where they can inflict major damage. Urinary schistosomiasis
causes scarring and tearing of the bladder and kidneys, and can lead to bladder
cancer. Intestinal schistosomiasis develops slowly, causing abdominal bleeding;
enlargement of the liver, lungs and spleen; and damage to the intestines. A
major indicator of the disease is blood in the urine and/or faeces. In those
who have been infected a long time, liver damage, kidney failure, infertility,
or bladder cancer may occur. In children, it may cause poor growth and learning
difficulty.</div>

We feel obliged to inform you of a disastrous and mind troubling incident. Dr Theresa is a Senior Registrar 1 in the department of Psychiatry of Obafemi Awolowo University Teaching Hospital (Ile-Ife). She is a very gentle, easy going, ever smiling and hardworking resident doctor.</div>

This incident occurred at her residence in the early hours of Thursday 25th February 2016 in her service apartment building situated outside the hospital where she resides. Dr. Okpara, [A Wing Commander in the Nigerian Air Force] who is a resident Doctor in the Dept of Community Medicine in the same hospital where Theresa works also resides in that compound.</div>

The parking lot within the compound is inadequate and as such tenants would have to occasionally call each other (through the security guard) to move their cars in the morning so that individuals are able to drive out. According to an eye witness (who is a doctor in the same hospital and also lives within the same compound), Dr Theresa's car was obstructing Dr. Okpara's car and he called her out to move her car. She had told him she was dressing up and would soon be out but he kept shouting and calling her. When she came out, he was still abusing her and being verbally aggressive. She then told him to take it easy since she had already told him she was dressing up and reminded him that he also does.</div>

As Theresa came out, entered her car and fastened her seat belt to drive out of the parking lot, Dr. Okpara continued barking asking her to move her car out, she told him she was already in the car and asked if he wanted to move it out himself? He then said he will teach her a lesson and went swiflty around the car, opened her car door and held her dress and punched her face several times. The Co-tenants had to run out to pull him away from her. At a point he reached for his metal pedal lock and was again restrained by tenants.</div>

Following the incident he showed no remorse, as he continued in his verbal aggression with his shirt buttons undone calling her names. He also dared her to call whoever in Abuja suggesting that he was above the law as she made attempts to make calls after the incident. Dr. Theresa was rushed to the hospital as being a known Sickle cell disease patient, her health was paramount at the time. On arrival, it was confirmed that indeed she had Cerebrospinal fluid leaking from her brain. This continued to be copious before ameliorating after some hours. In adition to the frontal bone fracture detected initially, a further depressed nasal bone fracture was detected on CT scan. Theresa now has to decide between going for surgery to correct the facial deformity or live with a disfigured face.</div>

Two doctor groups are reminding physicians to use antibiotics sparingly this cold and flu season.</div>

The American College of Physicians and the CDC urged doctors not to dole out antibiotics for colds, sore throats, bronchitis, and sinus infections that aren't complicated, according to guidance published in the Annals of Internal Medicine.</div>

"Reducing overuse of antibiotics for acute respiratory tract infections in adults is a clinical priority and a high-value-care way to improve quality of care, lower healthcare costs, and slow or prevent the continued rise in antibiotic resistance," Wayne Riley, MD, MPH, president of ACP, said in a statement.</div>

Antibiotics are prescribed at more than 100 million adult ambulatory care visits every year, and 41% of these prescriptions are for respiratory conditions, the guidance states.</div>

Antibiotic overuse contributes to the spread of antibiotic-resistant infections; in the U.S., there are 2 million antibiotic-resistant illnesses and 23,000 related deaths each year -- costing the healthcare system some $30 billion.</div>

Data from the CDC suggest that about 50% of antibiotic prescriptions are unnecessary or inappropriate, equating to more than $3 billion in excess healthcare costs, the guidance states.</div>

And antibiotics aren't without side effects. Indeed, they are responsible for the largest number of medication-related adverse events, implicated in 1 of every 5 emergency department visits for adverse drug reactions, the researchers said.</div>

To remind physicians about good antibiotic stewardship, ACP and CDC focused on four conditions that most likely will resolve on their own: the common cold, uncomplicated bronchitis, sore throats, and uncomplicated sinus infection.</div>

For uncomplicated bronchitis, physicians shouldn't conduct any tests, and should refrain from prescribing antibiotics, unless pneumonia is suspected. Patients can have cough suppressants, expectorants, antihistamines, decongestants, and beta agonists instead.</div>

<div style="text-align: justify;">
A sore throat should get analgesic therapy such as aspirin, acetaminophen, NSAIDs, or throat lozenges -- although testing can be done in patients with symptoms of group A streptococcal pharyngitis. That's the only case in which antibiotics would be warranted, the authors said.</div>

An uncomplicated sinus infection usually resolves without antibiotics, even in patients with a bacterial cause, the guidance states. Patients can have anaglesics for pain and antipyretics for fever.</div>

Antibiotics should be reserved for patients with symptoms lasting more than 10 days, if they start to develop severe symptoms or signs of a high fever (above 39°C/ 102.2°F), or if they have nasal discharge or facial pain that lasts three consecutive days.</div>

Adewole, who reiterated the capacity of the country to contain Lassa fever, explained that there were enough health professionals to manage the disease.</div>

He, however, denied claims by the Association of Medical Laboratory Scientists of Nigeria, that there were not enough diagnostic laboratories in the country.</div>

According to him, the Federal Government would establish new treatment centres for Niger, Bauchi, Niger and Taraba, Plateau, Ondo and Ebonyi states “in addition to the six that are active.”</div>

Adewole said, “We currently have testing capability in 14 testing centres some of which are in Maiduguri, Kano, Iddo, Irrua, Lagos, Port Harcourt and the FCT. We have treatment centres all over the country. We have enough personnel for managing Lassa fever. Unlike Ebola Virus Disease that is untreatable, Lassa fever is treatable. But we must start treatment on time to enable us to save the patients…”</div>

He emphasised that all the states in the country should be regarded as hotbed of Lassa fever.</div>

The minister said 17 states in the country were battling Lassa fever while 212 suspected cases in 62 local governments had been in existence since last year August.</div>

He also promised that the Federal Government would establish 109 Primary Healthcare Centres on Lassa fever in each of the six geo-political zones of the country in the next three months.</div>

Adewole said that government was determined to have a functional PHC centre in each ward across the country, and assured Nigerians that there was enough drugs to treat patients suffering from the ailment.</div>

He, however, warned that state governments should “not be under the illusion that the Federal Government alone can take care of the health needs of this nation because we must all drive it.”</div>

The Federal Government, he said, must not be the only one buying drugs for states, adding, “The Federal Ministry of Health cannot be producing everything. Health is on the concurrent list. We must do it together.”</div>

In his update on the disease, the Director of Nigerian Centre for Disease Control, Prof . Abudulsalami Nasidi, expressed fear that 50 per cent of all suspected Lassa fever cases in the country were not Lassa fever but might be symptoms of a new virus.</div>

Nasidi said, “We are worried whether we are dealing with another virus. The cases are different from Dengue, Ebola and Lassa fever,” Nasidi said.</div>

In his submission, the Niger State Commissioner for Health, Mustapha Jibril, called for the inclusion of traditional healers and health practitioners in the fight against the disease, as done in his state to stop the spread of the disease.</div>

However, residents of Osun State panicked on Tuesday following the death of a medical doctor at the Obafemi Awolowo University Teaching Hospital Complex, Ile Ife who was suspected to have died of Lassa fever on Tuesday.</div>

One of our correspondents gathered that the deceased, who returned from Ondo State with an illness, was admitted at the intensive care unit of the hospital before he died.</div>

He was said to have been vomiting blood and showing other symptoms associated with the disease before he died on Monday.</div>

A source at the hospital said, “The doctor just returned from Ondo State and he became ill. He was admitted on Sunday and he died on Monday. I was told that he showed symptoms similar to Lassa fever.</div>

“The hospital has started contact tracing and some of the nurse and workers have been placed under observation.”</div>

The chairman of a committee set up by the OAUTHC on Lassa fever, Prof. Adedeji Onayade, when contacted on the telephone by one of our correspondents said although a resident doctor died, the hospital had yet to confirm if he died of Lassa fever.</div>

Onayade said, “A resident doctor died and we are suspecting Lassa fever among other causes. We cannot say it is Lassa fever until test confirms it. “</div>

“We should not wait until there is an outbreak before we start rushing to address the epidemic.</div>

“This involves a lot of logistics which comes from the society, healthcare providers and the government itself.</div>

“It can be in the form of financing, appropriate equipment and facilities, well and adequately trained manpower to handle suspected and confirmed cases, and so on.</div>

“So whenever these are not adequate or properly applied, we have pockets and periods of epidemic of this nature,’’ he said.</div>

Regarding the death toll being higher than the case of Ebola, Askira said that the rodent transmitting the virus was endemic, while Ebola was imported to the country.</div>

The president called on all state branches of the association to take leading roles in safe guarding and protecting the health and lives of Nigerians.</div>

According to him, this can be achieved by setting up emergency teams in all the branches and collaborating with all federal and state health facilities to ensure effective and prompt referral networks, among others.</div>

baby after childbirth at the Rauf Aregbesola Health Centre in
Mosan-Okunola Local Council Development Area, Alimosho.<br />

Olulade expressed bitterness over the incident, saying, “We promise to
take swift steps in ensuring that such actions by erring personnel are
halted. Managements of public hospitals are urged to wake up to their
duties of providing effective health care in Lagos State.”</div>

The Commissioner for Health, Dr Jide Idris, told the meeting that the
government was committed to providing health care in its optimal
standard, particularly in the areas where high rates of maternal and
child mortalities were recorded.</div>

assure this gathering that the affected personnel would be dealt with
accordingly. I learnt that the doctor, who was supposed to be on duty,

I wish to begin this press briefing by wishing the citizens of Nigeria a
Happy and Prosperous New Year, and to allay their fears on the current
epidemic outbreaks of a Haemorrhagic Fever now confirmed by our
laboratories to be Lassa Fever Viral Disease.</div>
<div style="text-align: justify;">

The public is hereby assured that the government and its partners, and
other stakeholders are working tirelessly to address the outbreak and
bring it to timely end.</div>
<div style="text-align: justify;">
<br />
It is important that I notify the nation through you, that in the last 6
weeks Nigeria has been experiencing Lassa fever (LF) outbreak which has
so far affected 10 states. The States affected include Bauchi,
Nassarawa, Niger, Taraba, Kano, Rivers, Edo, Plateau, Gombe and Oyo
States. </div>
<div style="text-align: justify;">
The total number so far reported is 81 and 35 deaths, with a
mortality rate of 43.2%. Our laboratories have confirmed 17 cases,
indicative of a new roundtrip of Lassa fever outbreak.</div>
<div style="text-align: justify;">

The first case of the current outbreak was reported from Bauchi in
November, 2015. This was followed by cases reported by Kano State, and
subsequently the other states mentioned above.</div>

Lassa fever is an acute febrile illness, with bleeding and death in
severe cases, caused by the Lassa fever virus with an incubation period
of 6-21 days. Lassa fever was first detected in Nigeria 1969.<br />
<div style="text-align: justify;">
The number
of recorded cases peaked in 2012 when 1,723 cases with 112 fatalities
were recorded. It has continued to decline since then. About 80% of
human infections are asymptomatic; the remaining cases have severe
multi-system disease, where the virus affects several organs in the
body, such as the liver, spleen and kidneys. The onset of the disease is
usually gradual, starting with fever, general weakness, and malaise
followed by headache, sore throat, muscle pain, chest pain, nausea,
vomiting, diarrhoea, cough, and bleeding from mouth, nose, vagina or
gastrointestinal tract, and low blood pressure.</div>
<div style="text-align: justify;">

The reservoir or host of the Lassa virus is the “multimammate rat”
called Mastomys natalensis which has many breasts and lives in the bush
and peri-residential areas.</div>
<div style="text-align: justify;">
<br />
The virus is shed in the urine and droppings of the rats can be
transmitted through direct contact, touching objects or eating food
contaminated with these materials or through cuts or sores. Nosocomial
transmission also occurs in health facilities where infection prevention
and control practices are not observed. Person to person transmission
also occurs most especially when a person comes in contact with the
virus in the blood, tissue, secretions or excrements of an infected
individual.</div>

In response to these reported outbreaks, the Federal Ministry of Health
under my leadership has taken the following measures to curtail further

antiviral drug for Lassa fever to all the affected states for prompt and
adequate treatment of cases.</div>

ii.Deployment of rapid response teams from the Ministry to all the
affected states to assist in investigating and verifying the cases as
well as tracing of contacts.</div>
<div style="text-align: justify;">

iii.Clinicians and relevant healthcare workers had been sensitized and
mobilized in areas of patient management and care in the affected
states.</div>
<div style="text-align: justify;">

iv.Affected States have been advised to intensify awareness creation on
the signs and symptoms including preventive measure such as general
hygiene.</div>
<div style="text-align: justify;">

Furthermore, it is important to note that Nigeria has the capability to
diagnose Lassa fever. All the cases reported so far were confirmed by
our laboratories. However, due to the non-specific nature of Lassa fever
symptoms and varied presentations, clinical diagnosis is often
difficult and delayed, especially in the early course of the disease
outbreak.</div>
<div style="text-align: justify;">

In view of the major steps taken so far and the need to interrupt
transmission in the communities and our health facilities urgently, it
is important that I advise that the public is sensitized immediately
using electronic and print media. In addition, health educational
materials were distributed to all States, particularly the affected
areas for effective health promotion to enhance good “community hygiene”
including food hygiene and food protection practices. I urge all to
ensure that they avoid contact with rodents/rats as well as food/objects
contaminated with rats’ secretions/excretions. Avoid drying food in the
open and along the roadside. It is also important to cover all food to
prevent rodents’ contamination. Lastly, all suspected cases should be
reported immediately to enable early treatment.</div>
<div style="text-align: justify;">

Family members and health care workers are advised to always be careful
and avoid contact with blood and body fluids while caring for sick
persons with symptoms similar to those listed for Lassa fever and any
such patients should be taken to nearest health facility.</div>
<div style="text-align: justify;">

All health facilities in the country are hereby directed to emphasize
routine infection prevention and control measures, and ensure that all
patients are treated free.</div>
<div style="text-align: justify;">
<br />
Additionally, all health care workers should have a high index of
suspicion of Lassa Fever in patients presenting with febrile illnesses
particularly in endemic states, and all suspected cases should be
reported to NCDC/FMOH and the State Epidemiologists.</div>

On the basis of the effective control measures now instituted by the
FMOH, no travel restrictions will be imposed from and to areas currently
affected.</div>

Healthcare workers seeing a patient suspected to have Lassa fever should
immediately contact the State Epidemiologist in the state ministry of
health or call the Nigeria Centre for Disease Control and Federal

To support States and minimize impact and reduce mortality FMOH has
procured and distributed additional Ribavirin for immediate treatment
purposes and creation of strategic stock for future possible cases.</div>

confirmed, and I wish to seize this opportunity to express my sincere
gratitude to the WHO and other partners for the support so far.<br />

infections are asymptomatic; the remaining cases have severe

body, such as the liver, spleen and kidneys. The onset of the disease is
usually gradual, starting with fever, general weakness, and malaise
followed by headache, sore throat, muscle pain, chest pain, nausea,
vomiting, diarrhoea, cough, and bleeding from mouth, nose, vagina or

Due to the high level of illiteracy at the time of her birth, she was never diagnosed with sickle cell. She endured years of pain, which kept her away from school till she was 12 years old. Her pain was so severe and frequent that she would beg God to let her die.</div>

After primary school, she enrolled at Queens College Lagos, where she met her husband Bolaji Alakija, who later became a doctor. She did not know she had the sickle cell disease until after she had given birth to five of her six children.</div>

Her husband, Dr Alakija, gave her pills including folic acid to take every day, but kept the nature of her illness away from her for a long time.</div>

Dr Alakija had 10 wives and 27 children, when he died, she married Alhaji Laguda, but she never had a child with her second husband.</div>
